Data collected is the responsibility of Paul Temple and is used to provide counselling therapy online.
Why I collect your Data
I need to have certain information about you in order to engage in therapy sessions. As I am offering a service that involved both parties agreeing a working contract, it is necessary to collect information about you in order to do this. I will need to contact you to provide this service and your information will be used in order to do this. I will process payments for my service and your information is required to do this.
Data I will collect
In order to provide the service, I will collect the following date from you:
How is Data collected?
I will request that you complete an initial form when we agree to work together. The data on this form is used to establish service suitability. All data is requested by me and given voluntarily by you. I will not seek out additional data about you without your prior consent.
Where is Data stored?
Any data I collect from you will be stored electronically within password secure files. Only I have access to this data. Whilst data sent via the internet is not 100% guaranteed secure, I will take all reasonable steps to ensure your data is secure. Your contact details are held securely for 7 years after the therapeutic process has ended, at which point they will be deleted or destroyed accordingly.
Who is the Data shared with?
Data is viewed by myself. I will use clinical notes as part of my supervision requirements to discuss your case however your identity will never be revealed.
It may be necessary to contact your GP, or an emergency service in the event of an emergency. In this case I will have to reveal your personal date to assist the services as necessary. As part of your contract, you will have already agreed to this.
Complaints
In the event that you wish to make a complaint about the service you have been offered I would ask that in the first instance, you do so in writing.

- Your Date of birth
- Your full residential address
- Your email address
- Your telephone number
- An emergency contact number
- Your registered GP details
- Any relevant medical history
